Registration Requirements

Person enrolling and student must be residents of Paulding County to enroll in a Paulding County school. When registering a new student in the Paulding County School District and school is already in session, the student is expected to start school within 3 school days.
Only a parent/guardian living at the same address as the student may register and enroll the student. Step-parents cannot register a student on behalf of a biological parent. The biological parent that the student is residing with must register the student. If you have more questions regarding this issue, please contact Central Registration.
Documents required for registration to be completed:
- Acceptable proofs of residence
- Parent/Guardian state issued picture ID
- Birth Certificate or acceptable proof of age document for any special needs pre K, kindergarten or 1st grade student
- Georgia Department of Health Form 3231 - Certificate of Immunization (must be on the Georgia Form and completed by a Georgia doctor or Georgia health department) OR
- Notarized Georgia Department of Health Form 2208 - Affidavit of Religious Objection to Immunization (available at Central Registration) ONE PER STUDENT
- Georgia Department of Health Form 3300 - Certificate of Vision, Hearing, Dental, and Nutrition Screening (must be on the Georgia Form and completed by a Georgia doctor or Georgia health department). NOTE: All four sections must be completed for it to be accepted. OR
- Notarized Affidavit of Religious Objection to Required Health Screening Form 3300 (available at Central Registration) ONE PER STUDENT
- IEP for special needs students (highly recommended)
- Discipline records may be required before completing registration for students enrolling for 4th grade and higher.
Documents required within 30 days of registration:
- Birth Certificate (Grades 2-12)
- A second proof of residence if only one proof provided at time of registration
- Withdrawal Form/Grades / Transcript from previous school
