Statement of Legal Residence (2 pages)- one form per student
The Statement of Legal Residence is not proof of residence, but an additional form required if proof of residence is not in the parent's or spouse's name.
- If at least one acceptable proof of residence is not in the enrolling parent's name or their spouse's name, this form is required.
- Make sure to add the parent and spouse of the parent residing in the home with the student when completing the registration forms.
- This form is two pages AND must be notarized.
- One form per student is required
- Page 1 of the statement of legal residence must be completed by the person enrolling the student as the enrolling parent.
- Page 2 of the statement of legal residence must be completed by the person that is providing at least one acceptable proof of residence in their name.
- This form can be notarized at Central Registration at no charge or by an authorized notary public. Copies and emails of this form are not acceptable.
- The original notarized document must be received in person to be acceptable at the school if completing a change of address or at the Central Registration Office if registering a new student.
This form will be required to be renewed each subsequent school year before the first day of school. One valid current proof of residence must also be provided each subsequent year. This can be renewed at the school your student attends.
Acceptable proof of residence:
(1) First full page of monthly utility bill (power, water, cable, gas, garbage, home phone, home internet) or mortgage statement issued within the last 30 days which includes the service address. Remittance portion only is not acceptable. Two different utility bills can be provided as both proofs of residence (i.e. water and power or mortgage and gas). We must see the company's name/logo, bill holder's name, service/property address (not mailing address), and due or bill date.
- For ATT, must provide the page that has the bill holder's name and service address (at the top center), services being provided (middle of the page), and due date (side of page). This may a different page for each account depending on services and how bill is received.
- For Xfinity, must provide pages 1 and 3 as it has the bill holder's name, service address, services being provided, and due date.
- For Stream or Xoom Energy, must provide pages 1 and 2 as required information is on both pages.
- For trash, must provide the billing statement, NOT the payment receipt. The billing statement shows the months that the bill is for while the payment receipt only has a payment date (most have no year).
- For gas, must provide the billing statement of the company that you pay your bill to each month. Atlanta Gas Light is not acceptable as they provide the gas to your marketer (Gas South, GA Natural Gas, True Natural Gas).
(2) Lease or rental agreement signed by both parties consisting of written evidence that a current valid agreement exists. We must have pages that show the tenant's names, property address, term dates, and the signatures of both the tenants and landlord. Also to be included with this agreement shall be the name, address, and/or telephone number and email address of the landlord/management company. A current utility bill must be provided in a tenant’s name or the landlord’s name.
(3) Home ownership title consisting of either a warranty deed (pages 1-2 with Exhibit A if applicable), security deed (pages 1-4), or a quit-claim deed, to include physical address or legal property description.
