• Acceptable proof of residence:  Any two of the items listed below are acceptable

    (1)  First full page of monthly utility bill (power, water, cable, gas, garbage, satellite, etc) or mortgage statement issued within the last 30 days which includes the service address.  Remittance portion only is not acceptable. Two different utility bills can be provided as both proofs of residence (i.e. water and power or mortgage and gas).  We must see the company's name/logo, bill holder's name, service/property address (not mailing address), and due or bill date.

    1. For ATT, must provide page 3 as it has the bill holder's name, service address, services being provided, and due date.
    2. For Xfinity, must provide pages 1 and 3 as it has the bill holder's name, service address, services being provided, and due date.
    3. For Stream or Xoom Energy, must provide pages 1 and 2 as required information is on both pages.
    4. For trash, must provide the billing statement, NOT the payment receipt.  The billing statement shows the months that the bill is for while the payment receipt only has a payment date.
    5. For gas, must provide the billing statement of the company that you pay your bill to each month.  Atlanta Gas Light provides the gas to your marketer (Gas South, GA Natural Gas, etc).  They are not who you pay each month.

    (2)  Lease or rental agreement signed by both parties consisting of written evidence that a current valid agreement exists. We must have pages that show the tenant's names, property address, term dates, and the signatures of both the tenants and landlord.  Also to be included with this agreement shall be the name, address, and/or telephone number and email address of the landlord/management company.

    (3)  Home ownership title consisting of either a warranty deed, page 1-2 with Exhibit A, quit-claim deed, or pages 1-4 of security deed to include physical address.

    If the family is living in the household with someone else, a Statement of Legal Residence form is required AND must be notarized.  The original notarized document must be received in our office at 522 Hardee Street Dallas, GA 30132 by your reserved processing time.  We can notarize the form at our office if picture ID is presented and the person signing the form is present in front of us.  A copy of the form is not acceptable.  We must have the original notarized document.  For more information, please visit the Statement of Legal Residence page.
