• Kinship Caregiver Affidavit (pdf)- one per student

Kinship care refers to an arrangement where an adult is raising a child whose parents cannot or may be used for a biological father whose name does not appear on a student's birth certificate.  The adult providing care, whether relative or non-relative, has a relationship or bond with the child and the arrangement may be temporary or permanent.  It commonly refers to relatives raising children who are not in foster care.

A kinship caregiver is authorized to enroll a child in school by completing a Kinship Caregiver Affidavit.  The affidavit will expire at the end of the school year for which the affidavit was submitted, and must be submitted before the first day of school each school year.  The kinship caregiver will serve as the school’s point of contact for the child regarding truancy, discipline, and educational progress, while affidavit shall continue to be in effect.

Central Registration must have the original notarized Kinship Caregiver Affidavit before registration can be completed.  This can be notarized at Central Registration for no charge.
